Verify visible ingestion status, retrieve an answer with a source citation, then re-ingest a changed source under the same source key and inspect its version history. PASS requires retained original/normalized artifacts, citations with real available locations, and current-version retrieval. Invalidate a document to exclude it from current retrieval without deleting its history. Rebuild only derived indexing state from normalized content.
