# Phase 6: document knowledge

> Source-preserving ingestion, grounded retrieval, index rebuild, and recovery.

**Acceptance commit:** `c8169c8`. Phase 6 accepted PDF, DOCX, Markdown, and text ingestion; original and normalized artifact retention; versioning; citations; model-tagged 768-dimensional embeddings; rebuild; invalidation; corrupt-input visibility; cited owner correction; restart persistence; and isolated restore.

A deployed harness passed, a clean migration accepted revision `20260911_03`, and the 30-test Phase 4/5/6 suite passed. Current ranking uses stored compatible vectors plus cosine; pgvector remains provisioned rather than serving as the current nearest-neighbor path.
